SPORTS ACTIVITIES INVOLVE INHERENT RISK. PLEASE READ CAREFULLY.

Waiver and Release of Liability

In this agreement, the term herein referred to as “Activity” or “Activities”, includes all activities, events, or services provided, arranged, organized, conducted, hosted, provided, sponsored, or authorized by Canlan Ice Sports Corp., herein referred to as "Canlan"; and specifically includes, without limitation, ice, turf and court based sports, classes, camps, tournaments and all other activities, events and services in any way connected with or related to Canlan or its facilities, whether taking place before, during, or following participation in the Activities. The term “Participant” means the party, other than Canlan, who signs this agreement (or, in the case of a youth, the youth in respect of which a parent or legal guardian has signed this agreement), who shall be bound by the considerations set forth herein.

In consideration with being allowed to participate in any Activity, or an event related to an Activity, held at a Canlan facility, the undersigned acknowledges and agrees that:

1. ADULT PARTICIPANTS

The Participant understands and agrees that the Activities being engaged in have physical dangers that may result in serious injury and/or death. The Participant hereby acknowledges that such participation involves risks and hazards incidental there to, all of which are expressly assumed and hereby waives, releases, absolves and agrees to indemnify and save harmless Canlan, its directors, officers, employees and agents (including, without limitation, any contracted officials, staff, owners, or employees of any premise used by the Activity) of and from any and all actions, causes of actions, complaints, demands and claims whatsoever in existence prior to, on or after the date hereof whether in law or in equity, in respect of death, injury, loss or damage to the Participant or their property howsoever caused, except to the extent that the same are caused directly by the gross negligence of the aforementioned releasees. The Participant is advised to retain primary medical insurance benefits prior to participating in any Activity. The Participant certifies that they have no known medical condition that would prohibit them from participating in an Activity.

The Participant agrees to wear the specified equipment set forth in the event rules, and acknowledges that even with the recommended equipment, they are still at-risk to incur a minor or severe injury, up to and including death as a result of their participation in an Activity. The Participant agrees that they will act in a responsible manner while engaging in any and all Activities. The Participant agrees to reimburse Canlan, in full, within five days of notice, for the cost of any property damage for which the Participant is held responsible by Canlan. The determination of such responsibility shall be binding on the Participant, and the Participant agrees that there shall be no reimbursement to any Participant of fees paid to Canlan.

The Participant agrees to commit to fostering a safe, respectful, and inclusive environment for all participants, including athletes, coaches, staff, and spectators.

At Canlan Sports, our commitment is to provide positive sports experiences, and safety is integral to achieving this goal. All individuals engaged in Canlan Sports programs, tournaments, leagues, or events are responsible for fostering environments that are safe from physical, mental, and emotional harm. It is important for every participant, player, and guest to understand and abide by the Canlan Sports Participant, Player, and Guest Code of Conduct to maintain the integrity of sport and to ensure positive sports experiences for everyone.

You, as a participant, player or guest, are responsible, at all times, to act with:

RESPECT

You will treat teammates, opponents, coaches, game officials, spectators and Canlan Sports team members, both on and off the playing surface with respect, courtesy, and dignity. You will make sure all individuals feel welcomed, valued and encouraged to participate to the best of their ability. You are expected to treat our equipment and spaces with care, using them responsibly, and avoid intentionally damaging or misusing our property. You pledge to report any concerns or incidents of misconduct, abuse or unsafe behaviour to the appropriate authorities or Canlan Sports Management promptly and confidentially.

SPORTSMANSHIP

You will always promote fun and show good sportsmanship, which includes accepting both wins and losses gracefully, and refraining from taunting, or other unsportsmanlike behaviour. You will contribute to a supportive environment where you celebrate the achievements and successes of our fellow participants, fostering camaraderie, teamwork, and mutual respect. You respect the rules of the game and decisions of instructors, staff and game officials.

SAFE & FAIR PLAY

You will abide by the regulations of the sport and the spirit of the rules. You will compete fairly without resorting to cheating, dishonesty, or unethical tactics. You have a responsibility to prioritize the safety and well-being of yourself and others, and to avoid reckless or dangerous behaviour.

PERSONAL ACCOUNTABILITY

You will maintain self-control, discipline, and avoid behaviours that could harm yourself, your fellow participants, your team, or the reputation of the sport. You will cooperate, communicate, and show support for safe sport efforts. You acknowledge your mistakes, learn from them, and strive to improve as athletes and as individuals.

By adhering to this code of conduct, we ALL commit to creating a positive, inclusive, and enriching experience for all participants in sport. Together, we can foster a culture of safety, sportsmanship, fair play, and personal accountability that promotes the physical, emotional, and social well-being of everyone in our sports communities.

The Participant acknowledges that Canlan has the final authority and discretion to schedule the divisions and times in which the teams and Participants will play. The Participant understands and agrees that they may be suspended or permanently expelled from an Activity, or any of the following, but not limited to:

• Financial delinquency to Canlan
• Failure to abide by Activity rules
• Falsification of registration information

Consent to Filming

I consent to my participation in Activities being recorded through video camera, still camera and audio equipment and broadcasted to others for security, safety and/or commercial purposes. I understand and agree that I will not receive any remuneration in connection with the capture or public transmission of my participation in Activities at Canlan facilities. I understand and agree that such video footage may be transferred to, stored in or broadcasted in jurisdictions outside Canada and, thus, my personal information contained in such video footage may be accessible to law enforcement and/or regulatory authorities according to the applicable laws of such non-Canadian jurisdictions. I understand that I may withdraw my consent at any time, by contacting Canlan Sports Customer Contact Centre at contactus@canlansports.com, and that my consent will continue unless and until it is withdrawn.

1. YOUTH PARTICIPANTS

By my signature below, I, as the parent or legal guardian (in either case, the “Parent”), on behalf of a Participant in an Activity who is under the age of 18-years-old (known legally as a “minor”), give them permission to participate in all of the Activities contemplated herein, including, without limitation, sports, athletic programs, co-curricular Activities, outdoor education, off-site excursions and field trips.

The Parent understands and agrees that the Activities being engaged in have physical dangers that may result in serious injury and/or death to the Participant. The Parent hereby acknowledges that such participation involves risks and hazards incidental there to, all of which are expressly assumed and hereby waives, releases, absolves and agrees to indemnify and save harmless Canlan, its directors, officers, employees and agents (including, without limitation, any contracted officials, staff, owners, or employees of any premise used by the Activity) of and from any and all actions, causes of actions, complaints, demands and claims whatsoever in existence prior to, on or after the date hereof whether in law or in equity, in respect of death, injury, loss or damage to the Participant or their property howsoever caused, except to the extent that the same are caused directly by the gross negligence of the aforementioned releasees. I consent to my my child’s participation or the participation for a person that I am acting as legal guardian, in activities being recorded through video camera, still camera and audio equipment and broadcasted to others for security, safety and/or commercial purposes. Medical Release

In the event of illness or an accident while participating in any of the activity hosted or organized by Canlan, I understand it may be necessary to take my child, or a minor-aged participant who I’m legally bound to as a guardian, to a doctor or to a medical facility for treatment. Should this occur, I understand every attempt will be made to contact me at the telephone numbers I have

provided during the registration process for the activity. In an emergency or life-threatening situation, I understand that hospitals or other healthcare providers may be required to provide treatment without my consent.

Should a Canlan representative be unable to obtain my consent for treatment for any medical condition or accident, by my signature below, I authorize a representative of Canlan to act on my behalf by contacting our emergency contact names provided and/or by arranging for transportation, by emergency vehicle if necessary, to a hospital or other medical facility. Any doctor, transfer service, or medical facility may rely on this release to administer appropriate treatment or surgery necessary in the opinion of an attending physician.

I have read, understand, and agree to the above terms and conditions. I will also give permission for first aid trained and qualified Camp staff members to administer first aid treatment to my child and acknowledge that I will be responsible for any medical charges in connection with my child's treatment. III. ACCEPTANCE OF TERMS & CONDITIONS

I have read this release of liability and assumption of risk and fully understand the terms of this participant waiver for activities and events held at Canlan. I confirm that I sign this participant waiver freely and voluntarily of any inducements, and that no oral representations or statements have been made to me that would otherwise change, alter, or modify anything contained within this written agreement.
